CASE STUDY 1- ASTHMA
A 12-year old girl, Nina has been brought to Children's A&E by her mother with complaints of dyspnoea, wheezing, and chest tightness.
The history of the illness from the family includes a recent upper respiratory infection with cough, congestion and runny nose and a fever of 38.2 C.
Mum also states this has happened the last few times Nina had cold symptoms and she does seem to get relief from her salbutamol inhaler that was prescribed previously by her GP. Nina has not had her flu vaccine the last few years and usually only sees the GP when unwell.
Family/Social History
Family history revealed that Nina's Mum has asthma and because of this uses a lot of hand sanitiser for herself and family to keep them from "catching colds".
Nina lives with her mother, father and little brother age 6. Nina's dad is a smoker but mum states he smokes outside.
The family is living in a home currently under renovations. Assessment Upon assessment Nina has the following:
• intermittent coughing • expiratory wheezing • subcostal recession • temperature 38.2 C • heart rate 100 bpm • respiratory rate 40 • oxygen saturations 92 % (room air) • runny nose/nasal congestion • catching her breath often while speaking
Medications
She has been prescribed the following medications: • paracetamol 500mg po • salbutamol 5mg (nebulised) • prednisolone 30mg po
- Discuss the role of the CYP nurse in assessment and planning of care during illness, differentiating between the nurse's role and that of other health professionals
- Explain how clinical decisions and interventions are influenced by assessment showing appreciation that needs during illness are at times uncertain and will change requiring the need to evaluate and re-assess
- Compare and contrast the different types of knowledge utilised when making clinical decisions and planning care
- Discuss commonly encountered physical, mental, cognitive, behavioural health conditions and the nursing interventions to manage such conditions for CYP nursing practice
- Demonstrate the knowledge and skills required to plan care across the life span and in a range of clinical settings for CYP nursing practice
