Teaching and learning plan for adults patient with one of chronic disease ( asthma, HTN, or DM }at community

Develop a teaching plan for a group of clients with a common diagnosis that is seen in your clinical practicum placement. The teaching plan need to use evidence based practice guidelines (USPTF, or CDC) and treatment plans. The plan needs to include the following content:

2. Population demography
3. Setting
4. Content Outline -with reference list and citation within the text.
5. Methodology -include teaching aids.
6. Evaluation
   The Teaching plan is to be submitted with APA format must be used for any reference cited. The paper must be typed.
