• RCA Case study scenario:

You are a team of safety investigators. You have been tasked to determine the
Root causes that led to an accident (or a near-miss) within a given technology-based organization (THAT YOU SELECT).

• Referring to relevant knowledge from literature & course material:

General Definition of Root Cause Analysis (RCA): In science and engineering, root cause analysis (RCA) is a method of problem-solving used for identifying the root causes of faults or problems. It is widely used in various technology-based industries in fields such as IT operations, telecommunications, industrial process control, accident analysis (e.g., in aviation, rail transport, or nuclear plants), medicine (for medical diagnosis), healthcare industry (e.g., for epidemiology), etc.

Select a certain accident that took place within any sociotechnical organization (i.e.technology-based).

Get a good pool of information about that accident or serious incident, for
example by getting the formal published accident investigation report of that
accident, or from any other relevant resources.

Write a report in which you will list and explain or describe the root causes that
led to that accident or serious incident using any one of the very well established
root cause analysis tools. You can use HFACS taxonomy, fishbone diagrams, fault
trees, or any other similar root cause analysis tool.

o Clearly define the accident that you are going to analyze.
o Evidence of further free reading from peer-reviewed literature
(Note: accident investigators do refer to literature to derive their conclusions).
o Tables, figures, and illustrations to further support your answer, and make it more clear, strong, and more appealing. ( Try as far as possible to produce your own-created illustrations to express your ideas rather than copying from the internet.)
o The total main text part of the document should be around 2000 words
o introductions and conclusions must be included.
o List of references at end of document.
