Ask yourself: how would I try to preventive these high-priority failure modes, effects, and causes? Think carefully and creatively about changes that you would enact to the work system, so that (after these changes) the failure modes, effects, and their causes would occur less frequently and/or with less severity and/or be much easier to detect or eliminate before they happen.
Once you have imagined these Preventive Actions for each of the 5 high-priority rows, then document/describe them in the four remaining columns of your FMEA template. The four columns should contain the following:
- the Description column is where you should give details about what the preventive actions would be.
- the Owner(s) column is one or a few people (ideally as few as possible) who would be directly responsible for carrying out the preventive action. See the example_of_completed_FMEA.xlsxPreview the document for hypothetical examples
- the Due Date(s) column can be anything you think would be realistic. Assume that you are filling this out on the due date in the current year, so pick dates within the next few days/weeks/months after that.
- the Outcome Measurement Method(s) column is where you describe specific structure, process, and/or outcome measures that you would use to track progress toward completion of the preventive action(s) and/or the effects of the preventive actions. Although the column header says "outcome", you can include process or structure measures as well, if you think such measures would be helpful to tell if your preventive action is fixing the issue.
