Patient Story Interview

Identify a friend, family member or acquaintance and conduct a short interview about an experience with a healthcare system they have had as a patient, caregiver or family using the following questions to guide your interview. Provide a summary discussion posting on the discussion board below to analyze how this encounter met or did not meet IOM Quality Aims.
Describe the situation. Be sure to include relevant background information and include specific quotes from the patient/caregiver or family member. When did this situation occur? What happened?
Document how they responded to the situation. Use their own words when possible. Did they view this as a positive or negative? What was the impact on them?
Which of the IOM Quality Domains were either supported or violated in this situation?
What individual factors contributed to the impact of the situation on the patient/caregiver/family?
What systems issues contributed to the patient/caregiver/family experience?
What worked well? What kinds of improvements do you think are needed?
How did hearing about this situation make you think about your professional values and ethics?
IOM Six Quality Domains
Safe : Avoiding harm to patients from the care that is intended to help them.
Effective : Providing services based on scientific knowledge to all who could benefit and refraining from providing services to those not likely to benefit (avoiding underuse and misuse, respectively).
Patient-centered : Providing care that is respectful of and responsive to individual patient preferences, needs, and values and ensuring that patient values guide all clinical decisions.
Timely : Reducing waits and sometimes harmful delays for both those who receive and those who give care.
Efficient : Avoiding waste, including waste of equipment, supplies, ideas, and energy.
Equitable : Providing care that does not vary in quality because of personal characteristics such as gender, ethnicity, geographic location, and socioeconomic status.
