For the Needs Assessment assignment, you had to design a workplace assessment tool similar to a health
survey. For instance, this survey could have questionnaires for overall health history, self-assessment surveys
about mental and physical health, and a list of health program interests.
Most of what you will need to know for these assignments “CDC: Worksite Health Promotion” under
“Resources”. From here, click “Workplace Health Model”, and scroll down to the section that begins with
“Workplace Health Assessment”. This section should give you a good idea of how to approach these
assignments.
