Accessing the data:
- Go to the HHS Office for Civil Rights Breach Portal at: https://ocrportal.hhs.gov/ocr/breach/breach_report.jsf
- Use the filters on the left-hand side to select “Breaches affecting 500 or more individuals.”
- You can further refine your search by date range, covered entity type, breach category, etc.
2. Choosing visuals:
