Letter to a company officer

Write a letter to your company officer or a chief officer if you are a company officer addressing a situation where you or a peer could have been injured. In the fire service, these situations are commonly referred to as close calls, but solid situational awareness prevents a bad ending. You are required to explain the situation that occurred and identify recommendations to prevent it from happening again (e.g., training, equipment changes, or staffing issues). The body of your letter should be a minimum of 500 words.
