Health foundations Our focus so far this semester is to improve health outcomes and along the way, patient satisfaction and cost efficiency. Regardless in how we modify the system or the process by which the system operates, the patient remains the one consistent key to success. What we know is that all patients are not equal nor are they surround by the same environment: these conditions cause health disparities. For this assignment, you are to write an essay discussing health disparities and the influence the social determinants of health (SDoH) have on health outcomes. Start by defining health disparities and the SDoH. Address the impact they have on our health by illustrating specific examples using some of the data sources we have looked at earlier, e.g. BRFFS and County Health Rankings. Discuss ways in how the SDoH can be addressed to foster health promotion and improve health outcomes. Feel free to include tables, charts, graphs, or diagrams to make your case. Your essay should be approximately 1100-1500 words, written in 11 point font, single spaced, and submitted in Rich Text Format (RTF); include citations (no particular style) when warranted. Use your own words and if in doubt, cite, cite, cite. Overuse of quotations shows a lack of originality of thought. When using others ideas, thoughts or statistics, it is important to paraphrase the original and provide the appropriate citation. Quotations should be reserved for original thoughts that lose meaning when paraphrased or when the information comes from a person whose exact words hold meaning due to their title or position.
