1. Program and program strategies – briefly describe the program
   a. What were the program planners trying to accomplish (e.g. what health behaviors were they targeting? What do you believe their primary goal was for the program? What strategies did they use?)
2. Audience – describe the recipients of health behavior change (e.g. sociodemographic characteristics, ethnic/racial background, life cycle stage, disease and at-risk status)
3. Setting – describe where the health behavior change strategy is provided (e.g. school, community, worksite, health care setting, home, consumer marketplace, communication environment)
   b. Describe how the program planner used theory to guide/strengthen the program
   i. Program used a theory
4. What theory did the program planner use?
   a. You might want to informally ask/interview the program/intervention/ strategy facilitator.
   b. Remember, they might have also combined theories or models.
5. Was the theory a good fit? Justify why it was or was not a “good fit” by addressing the questions below:
   a. Was the use of that theory logical?
   b. Was the use of the theory consistent with what you observed?
6. Do you think the program planer should have used a different theory or parts of a different theory to strengthen the program?
