Evidence-Based Practice (EBP) Plan

Using the Quality Improvement model "Five Whys" you described in Week 3, provide a detailed explanation of the evidence-based practice plan that you will use to address the practice problem (HCAHPS question: During this hospital stay, how often did nurses explain things in a way you could understand?) (6–10 paragraphs). Be sure to include the sources you found in the analysis of the evidence to support your EBP plan. Use appropriate and persuasive language that communicates meaning with clarity and fluency to readers, and is virtually error-free.
