Choose to create a fictitious case study based on your own practice or personal experience (preferred option) or the Maryn case study below. If you use your own case study please ensure that you do so from a case management perspective, that is, within the role of a case manager assisting a person attending the service. Additionally, if you are using your own case study please provide a short one to two paragraph overview of the agency's and person's (client's) situation on the cover sheet of your assessment. This summary is not included in the word count for the task.
Whether you use either your own (specified on the cover sheet) case study or the Maryn case study, your task is to:
• Assume that you are the case manager. What is your initial professional understanding of the person in your case study (or Maryn’s) presenting need/s at the intake stage. Succinctly explain and justify your professional opinion.
• Describe your role as the case manager in assisting the person in your case study (or Maryn).
• Discuss how you would apply the overarching and phased functions of the case management model described in this subject and in the textbook, within the organisation and community context to your case study (or Maryn). This includes providing a reflective discussion on your approach to practice that focuses on decisions and reasons for the practice, the effectiveness of your practice and alternative approaches, skills or techniques (to demonstrate critical thinking).
• Demonstrate a person-centred case management approach to practice, with reference to the case study.
• Clearly outline and describe the communication skills you would use.
