Consider you being a nurse working the day shift on a busy postpartum unit.
You are assigned to care for Rachel a 22-year-old who delivered two twin boys last night. This is Rachel's first birth by cesarean section under anesthesia. Since Rachel has history of asthma and pneumonia and being smoker [smokes half pack cigarettes a day] she requires to use incentive spirometry q2hrs.
She was last medicated four hours ago with PO meds and states that her pain is currently 5.
Other objective findings include; VSS; incision is clean and dry; she has moderate lochia; her lungs are clear to auscultation. She is able to ambulate independently, and her Foley catheter was removed by the night shift staff nurse. She has not yet voided.
She takes oral antihistamines qd. Her asthma was well controlled during her pregnancy and has not smoked since she was admitted to the hospital. She is bottle feeding her boys.
She is married and works and teaching assistant at a middle school. She speaks both English and Russian.
You are required to create a teaching plan with Anna about using the incentive spirometer.
