Sample Answer
For my future role as an Advanced Practice Nurse (APN), a critical client-focused safety issue I have identified is Diagnostic Error.
While much focus is traditionally placed on medication safety, diagnostic errors—defined as the failure to establish an accurate and timely explanation of a patient’s health problem or to communicate that explanation—represent a significant threat to patient outcomes in primary and acute care settings.
Scope and Impact of Diagnostic Error
Diagnostic errors are often "silent" killers because they are harder to track than surgical or medication mistakes.
Prevalence: Research suggests that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. In outpatient settings, it is estimated that approximately 5% of adults experience a diagnostic error annually.
Clinical Impact: These errors lead to delayed treatment, unnecessary procedures, and psychological distress. In specialized fields, such as oncology or cardiology, a missed diagnosis (e.g., mistaking a myocardial infarction for acid reflux) can result in permanent disability or death.
