Scenario: You are working as a pharmacist and you are invited to submit a letter of intent (LOI) outlining a pharmacist led clinical service proposal designed to demonstrate the value of clinical pharmacy service(s).
Part 1: Select your patient population and clinical setting to implement your proposal
- The specific clinical pharmacy service is not restricted to only pharmacists (ex: can include physicians, nurses, social work specialists, etc) if you would like to propose a collaborative practice agreement, and could utilize students, residents, or technicians.
- The specific focus area is not defined and could include (but is not limited to) areas such as:
