A – Affiliations: What religious or spiritual groups do you belong to?
The patient, who I will refer to as Mr. Smith, is a 72-year-old man who was admitted to the hospital with pneumonia. He is a member of the Church of Jesus Christ of Latter-day Saints (LDS Church). He attends church services every Sunday and participates in activities at the church throughout the week.
D – Dietary Practices: Are there any dietary restrictions that I need to be aware of?
Mr. Smith does not have any dietary restrictions. He follows the dietary guidelines of the LDS Church, which are based on the principles of health and well-being. He eats a healthy diet that includes fruits, vegetables, whole grains, and lean protein.
